2026 Spring Boutique

Vendor Application Form / Fees $65 Per Space

Saturday, April 18, 2026 - 10 am to 3 pm

Please Note: Submitting an application does not guarantee a space, as vendor spots are limited. To create a well-balanced and enjoyable shopping experience, preference may be given to returning vendors and to those offering unique products. We also limit the number of vendors in similar categories (such as jewelry, soaps, clothing, baked goods, and related items).

Inside spaces includes either one 6 foot or 8 foot table and 2 chairs. Outside vendors must provide their own set-up, tents, chairs, tables.

Vendor Fee & Raffle Prizes

To simplify the process for everyone, the vendor fee this year is $65. This allows us to purchase raffle prizes directly, so vendors are not required to donate a raffle item. We hope this makes participation easier and allows vendors to focus on selling and showcasing their products.

Vendors will be notified by email if accepted. Once accepted, vendor fees are due immediately to secure your space.

RELEASE AND WAIVER OF LIABILITY

AND INDEMNITY AGREEMENT

In consideration of being permitted to participate as a vendor in an event hosted by the Covina Woman’s Club at 128 San Jose Avenue, Covina, California 91723 (the “Premises”), the undersigned (“Vendor”) hereby releases, waives, discharges, and covenants not to sue the Covina Woman’s Club, including but not limited to its officers, members, volunteers, and agents (collectively, the “Releasees”), from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death, whether to person or property, whether caused by the negligence of the Releasees or otherwise, while the Vendor is in, on, at, or about the Premises or using any facilities or equipment thereon.

Indemnification and Hold Harmless

The Vendor agrees to indemnify, defend, and hold harmless the Releasees from and against any and all loss, liability, damage, cost, or expense (including reasonable attorneys’ fees) incurred as a result of the Vendor’s presence on the Premises or participation in any Covina Woman’s Club activity, whether caused by the negligence of the Releasees or otherwise.

Assumption of Risk

The Vendor voluntarily assumes full responsibility and all risks of bodily injury, death, or property damage arising from any cause, including but not limited to the negligence of the Releasees, while in, on, at, about, or upon the Premises or while participating in any Covina Woman’s Club event or activity.

Severability

This Agreement is intended to be as broad and inclusive as permitted by the laws of the State of California. If any provision is held invalid or unenforceable, the remaining provisions shall continue in full force and effect.

Electronic Signature & Acknowledgment

The Vendor acknowledges and agrees that:

• This Agreement is being executed electronically

• An electronic signature, checkbox acknowledgment, or typed name constitutes a legally binding signature

• The Vendor has read, understands, and voluntarily agrees to all terms of this Agreement

• No oral representations, statements, or inducements outside of this written Agreement have been made

This Agreement shall be effective as of the date of electronic execution.
