I understand that inclusion on the Covina Woman's Club Preferred Vendor List is at the sole discretion of the Covina Woman's Club and is not guaranteed.

I understand that proof of current business liability insurance will be required before being approved as a Preferred Vendor.

If selected, I agree to conduct business in a professional, courteous, and ethical manner and to provide quality service to clients.

If selected, I authorize the Covina Woman's Club to list my business name, logo, contact information, website, and social media links on its website, marketing materials, and social media for the purpose of promoting the Preferred Vendor List.

I understand that the Covina Woman's Club may remove my business from the Preferred Vendor List at any time.
